Your first appointment at our office is a key component of your treatment. Your consultation appointment is your opportunity to ask any questions you may have about treatment and discuss your anesthesia options. You will also receive your pre- and post-operative instructions, which will help you prepare for surgery and recovery. In addition, your first appointment gives Dr. Daniel a chance to get to know you and get a thorough look at your condition. He will take any necessary X-rays and/or 3D scans, review your health history, and talk to you about your needs. All of these factors will help him develop a comprehensive and personalized treatment plan. At your first visit, it’s vital that you bring
- Valid photo identification
- Referral slip (if you have it)
- Any previously taken X-rays or 3D scans
- List of medications you currently take
- Valid medical card
- Insurance information for billing
Note: Patients under the age of 18 must have a responsible adult accompany them to the office and remain with them during the appointment.
We do our best to stay on schedule and get all of our patients in on time, which is why we ask that you arrive a few minutes early. This gives you a chance to fill out any remaining paperwork without getting behind schedule.
